NEW CLIENTS:
- Bring a copy of your income tax return from last year
- Bring Social Security Cards for each family member and dependent
- Bring a list of dates of birth for each family member and dependent
INCOME DOCUMENTS
- W-2’s, SOCIAL SECURITY #’S, DOB for yourself and all dependents
- 1099-MISC, 1099-R, 1099-G
- Proof of jury duty pay
- Proof of alimony you received
- Social security statement (1099SA)
- Dividend and interest statements (1099-DIV AND 1099-INT)
- Retirement Account distributions (1099-R)
- Brokerage statement (1099-B) along with statement showing when you bought and sold
your investments - K-1 statement reporting profits from partnerships, trusts, small businesses, and estates
- Record of income and expenses for your rental property
- Record of income and expenses for your self-employment (1099 NEC)
OTHER TAX DOCUMENTS
- Settlement statements (HUD) if you purchased a home or refinanced
- Summary of moving expenses if job related
- Summary of educational expenses (College tuition 1098T)
- Summary of your childcare, day care, or adult day care expenses
- IRA contributions (traditional contributions (Traditional, SEP or Rollovers)
- Student loan interest paid (1098-E)
- Health insurance form (Mass-HC1099) (Federal-1095A,1095B,1095C)
- Amount of stimulus check (Form 6475 / 1444-C) or Advanced Child Tax Credit payments. (Form 6419), (if applicable)
TAX DEDUCTION DOCUMENTS
- Health care expenses (doctors, dentists, health insurance, co-pay, eye care, prescriptions)
for yourself or your family members that you paid - Real estate taxes
- Mortgage interest
- City or town excise tax
- Gifts to charity (check, credit card statement, letters or receipts for clothing, household
items, etc. from charity - MA ONLY (educator expenses, MBTA commuter expenses)
- Loss of property due to casualty or theft
- Gambling losses (W-2) – only if you have winnings (1099-G)
- Apartment or land rent
